All you have to do is fill out the request form and allow 3-6 weeks for delivery. Limit one item per household or address. Please note: You will get a pop window asking for your phone number – enter all zeros – you will get another pop up window asking for a valid phone number – just “X” out of the pop up window and you will see a screen that states your request has been processed. Please allow 3-6 weeks for delivery.
